Required Forms for the Summer Institute
All Students must complete the Scholar Summer Institute Forms Packet and submit by the established deadline in order to particpate in the Upward Bound & Upward Bound Math-Science Summer Institute. Forms must be submitted yearly to abide by Federal Requlations. As you complete the forms online, be sure that you are completing each section accurately and completely.
Student Contact Information - Ensures accurate and up-to-date demographic information for compliance. Note: Parent/guardian email is essential to facilitate communication between Rutgers Upward Bound staff and student's family.
Emergency Contact Form - Informs staff of who to contact during a student emergency, whether offsite or in the residence hall.
Guest and Student Pick-Up Pre-Registration Form - Allows Parent(s)/guardian(s) authorization to approve student's guests and person(s) designated to pick up the student. All guests, including family members, must be listed on the form. All guest and authorized pick-up persons must provide a valid form of photo identification when asked by Upward Bound & Upward Bound Math-Science staff.
Must be printed and returned to RUB Offices:
Rutgers Health Services Permission to Diagnose and Treat - Gives authorization for a student to be treated by Rutgers Health Services should that student become ill or injured during the Summer Institute.
Consent Waiver - Gives authorization of student by parent/ legal guardian to attend all trips & activities during the summer & academic year. (Must be printed and returned)
Photo/Video Release: Gives authorization of student by parent/legal guardian to participate in video and photos during programmings and events. (Must be printed and returned)
Summer Food Eligibility Application: All scholars must complete. Gives authorizatin to participate in NJ Summer Food Program. (Must be printed and returned)
Only For Newly Admitted Students Please complete the Immunization Form
- All students who reside and/or take classes at Rutgers must undergo a full medical immunization. The corresponding form must be completed, signed by a medical doctor, and submitted yearly.
